Application for Adoption

Most of our babies are rescued from owner surrenders or referrals from vet and/or calls from concerned animal lovers.  If you have specific criteria as to sex, color, or coat type, this is not the rescue for you. I have purebreds and mixes some with papers and some not.  Your focus of rescuing a dog from an unhealthy or cruel environment should be the reason you are completing this application.  Remember, these are dogs that require patience, understanding and a willingness from their new owner to help them adjust. Some have been treated very badly and some have been shoved aside like an old used rag. I have had them spayed/neutered and heartworm checked and on meds. All are on K9 Advantage Plus and have had all shots to current time. Any new arrivals are given the same care.  This is a life long commitment to one of the best friends you may ever have.  Are you willing to make that commitment?

Ellen’s Rescue reserves the right to refuse or deny any application.  By signing and dating below, you agree that all the information listed on this application is complete and truthful.  Also, by signing and dating below, you authorize Ellens Rescue to verify the information on this Application for Adoption.  Any misrepresentation will be grounds to automatically reject the application.

Here’s how we work.  Upon receipt of this application, you will be contacted by a representative for an in-depth interview.  If this interview indicates your life circumstances, etc. are not a good match for our rescued dogs, the interview process will stop.  If this interview indicates further information, your references and vet will be contacted.  If possible, a home visit will be arranged but this is not always mandatory.  Then a determination will be made whether to accept or deny your application.  You will be notified either way within two weeks.  Please remember, our organization is dedicated to making sure our rescues placed in the home best suited for its temperament and future development.  Thanks for your time in completing this form. 

If you are approved and adopt one of our rescued dogs you will be required to complete an Adoption Agreement, and travel to where the rescue is being fostered).  Our adoption fee is based on amt. needed to cover vet bill and  includes the following:  a well puppy visit, all necessary age-appropriate vaccinations and spay/neuter if over six months of age, rabies vaccination, heartworm test, bordatella shot.
